with regards to blotted out name at the end page, i have been trying to rearrange my name formally, but it turned out more difficult than expected with regards to 'legality' or 'law'. therefore, until they correct the application form in this country, i intend to simply write, and then cross out my earlier name, where necessary, then they may learn to not make the application for name change such an impossible process here (mainly i hoped to rid myself of the first and last names, to remain the 2 middle names - it is sometimes difficult to abide by such "law"). have a happy day
